Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submitted Two (2) References Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference. The name of the facility and the dates of employment must match the resume. Reference must include a breakdown of skills and strengths with a rating score.
