We are seeking a PRN Occupational Therapist (OT) to join our home health team. In this role, you will be responsible for conducting in-home evaluations and delivering high-quality rehabilitative care to adult patients, helping them regain independence in their own environment.

Key Responsibilities Patient Evaluations:

Conduct comprehensive in-home adult patient evaluations to develop and implement individualized plans of care.

Interdisciplinary Collaboration:

Maintain proactive communication with patients, family members, physicians, and other healthcare professionals to ensure a holistic approach to recovery.

Documentation:

Utilize online systems to complete timely documentation, ensuring full compliance with federal, state, and company policies.

Travel:

Provide services "on the road," traveling to various patient residences within the designated service area.

Qualifications & Requirements Experience:

Home Health or Nursing Home:

1 year (Preferred).

License & Certifications:

State Licensure:

Valid Texas Occupational Therapist License (Required). >
