$70.00•$75.00 per hour (Hourly)
Pay:
Weekly pay with direct deposit. BrightStar Care is seeking a licensed Occupational Therapist to provide oneonone home health services to patients in their homes. The Occupational Therapist evaluates patients, develops individualized treatment plans, and helps patients regain functional independence, safely perform ADLs, and improve overall quality of life. Responsibilities:
Evaluate patients and develop individualized treatment plans based on physician orders and clinical assessments. Assess patients ability to perform ADLs (dressing, bathing, grooming, meal prep, household tasks). Develop interventions to improve mobility, coordination, strength, cognition, and independence. Recommend adaptive equipment, assistive devices, and environmental modifications. Educate patients, families, and caregivers on therapeutic techniques, safety, and equipment use. Identify safety concerns and fall risks in the home. Monitor patient progress and adjust treatment plans to meet goals. Collaborate with physicians, nurses, caregivers, and other healthcare professionals. Communicate changes in condition or treatment response to physician and Director of Nursing. Complete accurate documentation, progress notes, and discharge summaries. Treat patients, families, caregivers, and staff with professionalism and respect.
Qualifications:
Masters Degree or Doctorate in Occupational Therapy (required). Current Occupational Therapist license in the state of practice. Successful completion of all staterequired licensing standards. Minimum of two (2) years OT experience. Home health or communitybased experience (preferred). Strong clinical assessment, communication, and documentation skills. Commitment to HIPAA compliance and patient confidentiality.
