Evaluate patients and develop individualized treatment plans Provide skilled occupational therapy services in the home Help patients improve independence with daily activities Educate patients and caregivers on therapeutic techniques and home programs Monitor patient progress and modify treatment plans as appropriate Complete required clinical documentation Coordinate care with physicians, PTs, nurses, and other healthcare professionalsRequirements:
