Home Health OT North Fort Worth & Mid Cities:

Hurst, Euless, Bedford, Grapevine, Southlake, Keller) We are seeking a licensed Occupational Therapist (OT) for a PRN/Contract home health position to provide high-quality, patient-centered therapy in the comfort of patients' homes This role offers exceptional flexibility, allowing you to manage your own schedule. You will evaluate patients, develop customized care plans, and deliver therapeutic interventions aimed at helping adult and geriatric populations regain independence in their daily living activities. You will be completing Evaluations, OT visits, Re-assessments, Recerts and Discharges as well as supervise a COTA. This is a PRN / contract opportunity ideal for therapists looking to increase income while maintaining full control of their schedule.

Patient Evaluation :

Conduct comprehensive home assessments to evaluate patients' functional capabilities, cognitive status, ADL performance and home safety hazards.

Care Planning :

Develop and implement individualized occupational therapy care plans based on physician orders and evidence-based clinical goals.

Therapeutic Intervention :

Deliver hands-on training for activities of daily living (ADLs) including bathing, dressing, grooming, and transfers.

Adaptive Equipment :

Recommend, procure, and train patients and caregivers on the safe use of adaptive equipment, durable medical equipment (DME), and compensatory strategies.

Caregiver Training :

Educate families and caregivers on proper transfer techniques, fall prevention strategies, and home exercise programs.

Documentation :

Complete precise clinical notes, assessments (such as OASIS, if applicable), and discharge summaries using electronic medical records (EMR) software (Kinnser/ Homecare Homebase) within required timelines.

Care Coordination :

Collaborate closely with physicians, physical therapists, and nurses to ensure cohesive patient care.

Education :

Master's, or Doctorate degree in Occupational Therapy from an ACOTE-accredited program.

Licensure :

Active, unrestricted license to practice as an Occupational Therapist in the state of Texas.

Certification :

Current Basic Life Support (BLS) / CPR certification.

Experience :

Minimum of 1-2 years of clinical experience as an OT is preferred; prior experience in home health care or adult physical rehabilitation is highly preferred.

Compliance :

Valid driver's license, reliable personal vehicle, and active auto insurance for traveling to patient homes. Must carry own personal liability insurance.

Skills :

Strong clinical reasoning, excellent time management, autonomy in independent settings, and proficiency with mobile EMR documentation.

Pay:

$65.00 - $80.00 per hour
