Full Time and/or PRN Occupational Therapist (OT) - In-House Rehab | Midland, TX Focused Post Acute Care Partners (FPACP) is a dynamic company with an expectation of excellence in the delivery of long-term care to every patient every day. Position Overview As an Occupational Therapist (OT), you will evaluate, plan, and implement evidence-based treatment programs designed to improve residents' independence, safety, and overall quality of life. You will play a key role in interdisciplinary collaboration, patient and caregiver education, and delivering high-quality rehabilitative care. Key Responsibilities Evaluate residents to determine functional impairments and therapy needs Develop and implement individualized treatment plans focused on ADLs and IADLs Monitor patient progress and adjust treatment plans as needed in collaboration with the interdisciplinary team Recommend adaptive equipment and environmental modifications to promote safety and independence Educate patients, families, and caregivers on therapeutic techniques and functional strategies Maintain accurate and timely documentation in accordance with state and federal guidelines Participate in care planning meetings and team collaboration Ensure compliance with all regulatory and facility policies Qualifications Active Texas Occupational Therapist (OT) license In good standing with all relevant licensing boards and regulatory agencies Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred Familiarity with Net Health documentation system a plus Strong interpersonal, communication, and clinical skills Why Join Us?
