Job Description
Provide skilled Occupational Therapy services on an intermittent basis to patients in their homes in accordance with physician orders. Provide initial and ongoing comprehensive assessments of the patient's needs. Establish a treatment plan in consultation with the physician. Instruct the patient and family and assist in teaching exercise programs to the patient and family. Following Therapy plan of Care. Perform routine strengthening and therapeutic exercises. Home Health experience is a plus!
