2992011545-2 Apply for this job now Job Description Adventist Health is seeking a compassionate Occupational Therapist to provide home-based care that promotes safety, independence, and whole-person wellness. In this faith-based, mission-driven role, you will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions in patients' homes. You'll educate patients and caregivers, recommend adaptive equipment, and collaborate closely with an interdisciplinary team. This position offers the opportunity to impact community health while growing professionally in a supportive, values-centered environment. Responsibilities Conduct home-based evaluations to assess functional abilities, safety, and environmental needs. Develop and implement individualized care plans to improve ADLs, mobility, and independence. Deliver evidence-based therapeutic interventions and home exercise programs. Educate patients and caregivers on safety, equipment use, and strategies for daily living. Collaborate with nurses, physicians, and the interdisciplinary team on coordinated care. Document assessments, goals, progress notes, and outcomes in the EHR in a timely manner. Recommend and train in use of adaptive equipment and home modifications. Support whole-person care by addressing physical, psychosocial, and spiritual needs in line with Adventist Health's mission.
