Job Overview We are seeking a dedicated Occupational Therapist to provide comprehensive home health services. In this role, you will assess patients' needs, develop personalized treatment plans, and deliver physical rehabilitation therapy to support individuals with disabilities and promote their independence. The ideal candidate will have experience working with diverse populations and a strong commitment to improving patient outcomes through skilled occupational therapy interventions. Duties Conduct occupational therapy assessments to evaluate patients' physical, cognitive, and emotional needs. Develop individualized treatment plans tailored to each patient's goals and medical conditions. Deliver effective physical rehabilitation occupational therapy in a home health setting, utilizing evidence-based practices. Implement therapeutic interventions aimed at improving daily functioning and quality of life for patients with disabilities. Perform clinical documentation accurately and timely, ensuring compliance with healthcare regulations and standards. Collaborate with patients, families, and healthcare teams to coordinate care and monitor progress. Educate patients and caregivers on therapeutic exercises, adaptive techniques, and safety measures to support ongoing recovery. Experience Proven experience in occupational therapy, preferably within a home health or community-based setting. Demonstrated expertise in conducting occupational therapy assessments and developing treatment plans. Background in physical rehabilitation occupational therapy is highly desirable. Prior experience working with individuals with disabilities, including understanding their unique needs and challenges. Strong clinical documentation skills to maintain accurate records of patient care and progress. Pay:

$50.45 - $70.00 per hour Expected hours: 20.0 - 40.0 per week
