Oasis Therapy Services is seeking an Occupational Therapist (OT) to provide one-on-one rehabilitation services in a assisted living setting in Evansville, Wisconsin.

Net Health Responsibilities Evaluate residents and develop individualized treatment plans. Provide therapy focused on ADLs, mobility, safety, and independence. Monitor progress and adjust treatment plans as needed. Educate residents and caregivers on exercises, adaptive techniques, and equipment. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Wisconsin Occupational Therapist license. Graduate of an accredited Occupational Therapy program. Current CPR certification. Strong communication, organization, and documentation skills.
