Renewal Rehab is seeking a Licensed Occupational Therapist to provide compassionate, evidence-based care for seniors in skilled nursing and post-acute settings. In this role, you will evaluate patients, develop individualized treatment plans, and deliver interventions that promote functional independence, safety, and quality of life. You'll collaborate closely with an interdisciplinary team, document outcomes, and educate patients and families. Responsibilities Perform comprehensive occupational therapy evaluations for patients in skilled nursing and post-acute care settings Develop and implement individualized, evidence-based treatment plans focused on functional independence and safety Provide therapeutic interventions including ADL training, cognitive and neuromuscular re-education, and therapeutic activities Document evaluations, treatment sessions, and progress in the EMR accurately and timely Collaborate with an interdisciplinary team, including PT, SLP, nursing, and physicians, to coordinate patient care Educate patients, families, and caregivers on home programs, safety strategies, and adaptive equipment use Participate in care conferences and contribute to discharge planning and goal setting Maintain compliance with state licensure, professional standards, and facility policies Engage in mentorship, continuing education, and clinical development opportunities Support a collaborative, patient-centered culture aligned with Renewal Rehab's mission and values Required Skills Occupational therapy evaluation Treatment planning Geriatric rehabilitation ADL and functional training Neuromuscular re-education Cognitive and safety training Therapeutic exercise and activities Care planning and documentation (EMR) Interdisciplinary team collaboration Patient and family education
