Job Description

Renewal Rehab is seeking a Licensed Occupational Therapist to deliver patient-centered care for primarily senior populations in skilled nursing and post-acute settings. In this role, you will perform comprehensive evaluations, develop individualized treatment plans, and provide evidence-based interventions to improve function, safety, and quality of life. You'll collaborate closely with an interdisciplinary team, document in EMR, educate patients and families, and contribute to outcome-focused programs. Responsibilities Perform comprehensive occupational therapy evaluations for primarily senior patients in skilled nursing and post-acute settings Develop and implement individualized, evidence-based treatment plans focused on function, safety, and quality of life Provide therapeutic interventions including
ADL/IADL
training, functional mobility, balance, and cognitive strategies Collaborate with an interdisciplinary healthcare team to coordinate and optimize patient care and discharge planning Document evaluations, treatments, and progress accurately and timely in the EMR system Educate patients, families, and caregivers on home programs, safety strategies, and equipment needs Monitor patient outcomes and adjust treatment plans to meet clinical goals and regulatory requirements Participate in clinical mentorship, continuing education, and quality improvement initiatives Support a collaborative, patient-centered culture aligned with Renewal Rehab's mission and values Required Skills Occupational therapy evaluation and treatment planning Geriatric rehabilitation
