Job Description
Overview Join our dynamic Cognitive Rehabilitation program as a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW), where you will play a vital role in supporting individuals with cognitive challenges. This energetic position offers an exciting opportunity to make a meaningful impact through evidence-based practices and compassionate care. You will work closely with clients across various settings, including outpatient clinics, telehealth platforms, and inpatient facilities, to promote recovery, resilience, and overall well-being. Your expertise will help empower clients on their journey toward improved cognitive ability and functional independence. Training will be available and mandatory. Duties Develop personalized treatment plans incorporating cognitive rehabilitation treatments with cognitive behavioral therapy, and other evidence-based practices tailored to individual goals. Provide direct counseling services including individual and group for individuals with cognitive decline. Utilize telehealth platforms to deliver clinical counseling and crisis intervention services while maintaining strict adherence to HIPAA regulations and clinical documentation standards. Engage in case management activities such as patient care coordination, medical records documentation, and treatment planning to ensure comprehensive support. Apply advanced skills in counselling and cognitive rehabilitation treatment and ICD coding for precise clinical documentation. Collaborate with multidisciplinary teams on inpatient and outpatient cases to optimize patient outcomes through research-informed practices and data collection. Requirements Valid Colorado licensure as an LPC or LSW with eligibility to practice independently in the relevant state. Proven experience working with individuals with disabilities or memory loss in clinical or outpatient settings. Strong knowledge of mental health assessments, diagnostic evaluation processes (ICD-9/ICD-10), treatment planning, and clinical documentation standards. Familiarity with telehealth therapy platforms and electronic health records (EHR/EMR) systems such as Epic or Cerner. Competence in crisis management techniques including crisis intervention and trauma-focused therapy for diverse populations. Excellent communication skills for client engagement and interdisciplinary collaboration; ability to handle sensitive situations with compassion and professionalism.
