Job Description

The Rehabilitation Specialist (RS) is responsible for providing the clinical services to the clients of C&C Advocacy and Health Services, Inc. They are also responsible for supervising the direct care workers and ensuring that the clients are receiving the appropriate services as outlined in their Individual Rehabilitation Plans. The RS also assists the Program Director with providing the necessary training for direct care workers. The RS reports to the Program Director, their specific responsibilities are as follows: Responsibilities and Duties Complete initial screenings for all individuals referred to the program. Complete client assessments for all individuals entering the program. Review diagnostic impressions and ensure validity of diagnosis with client presentation, collaborating with referring therapist and documenting if changes have been observed in client's clinical presentation. Ensure that each client has a current and comprehensive individualized rehabilitation plan. Ensure that all client contact is documented appropriately in the form of session notes. Ensure that each client has a current and comprehensive rehabilitation monthly progress report. Ensure ongoing communication and coordination of services between the mental health service provider and the rehabilitation service provider. Supervise service delivery to ensure the effective implementation of treatment and case management services. Supervise the completion of clinical documentation. This includes ensuring documentation is completed in a timely manner and its content is of sufficient quality. In addition, ensure that the clinical treatment flow is clear, comprehensive, and observable in the client file. Supervise the clinical service delivery to ensure effective implementation and execution of clinical interventions and clinical supervision. Participate in monitoring the revision and continued development of policy and procedures in collaboration with Program Director. Participate in facilitating the development and revision of forms and tools utilized to administer rehabilitation services in collaboration with the Program Director. Facilitate group and individual supervision meetings with direct service workers. Attend Management Meetings. Facilitate In-Service Training once monthly for direct care workers in collaboration with the Program Director. Ensure medical compliance is documented on appropriate forms for maintenance of health passport and forward medical information to appropriate parties. Ensure compliance and securing medical attention for the overall well being of clients in care. Coordinate team meetings to develop rehabilitation plans for clients and ensure implementation of treatment services Document clients' progress in monthly treatment summaries. Coordinate with CSA and other outside agencies for effective and efficient delivery of services. Broker community resources and services to meet clients' needs. Maintain client files with appropriate medical, educational, and treatment records and facilitate correspondence with inter-agency team members. The Rehab Specialist will be solely responsible for the supervision of the direct care staff per COMAR regulations.
Qualifications:
Minimum
  • licensed mental health professional required; minimum of two years of experience providing direct care services to emotionally disturbed youth as a rehabilitation specialist supervising direct care staff; knowledge of applicable state and regulatory guidelines set for operations of psychiatric rehabilitative services.
