In this vital role, you will provide comprehensive therapeutic and support services to individuals with diverse mental health needs. Your expertise will help clients achieve greater independence, improve their quality of life, and foster positive growth within their home and community environments. This position offers an exciting opportunity to make a meaningful difference through compassionate care, innovative therapy techniques, and collaborative case management. Responsibilities Conduct thorough patient assessments and intake interviews to determine individualized treatment plans. Implement behavior management strategies and crisis intervention techniques to ensure safety and promote positive behavior change. Provide direct support and counseling for children, adolescents, adults, and families dealing with mental health issues, developmental disabilities, or substance use challenges. Collaborate with multidisciplinary teams on patient care plans, social work initiatives, special education programs, and hospital or community-based services. Qualifications Active LMSW license required Experience working with mental health and community-based services preferred Strong documentation and case management skills Ability to collaborate with PRP staff, clients, therapists, and community providers Strong background in behavioral health counseling, mental health therapy, or clinical social work. Familiarity with crisis management protocols and crisis intervention strategies in diverse settings.

$35.00 per hour
