POSITION SUMMARY
The Rehabilitation Specialist provides rehabilitative and programmatic oversight for The Crossroads' adult Psychiatric Rehabilitation Program. Working collaboratively with program leadership and direct-care rehabilitation staff, the Rehabilitation Specialist helps ensure that services are person-centered, recovery-oriented, appropriately documented, and delivered in accordance with applicable Maryland regulations, Medicaid requirements, accreditation standards, payer requirements, and organizational policies. The Rehabilitation Specialist is responsible for rehabilitation assessments, Individual Rehabilitation Plans (IRPs), DLA-20 assessments, documentation review, staff supervision and education, quality assurance, participant progress monitoring, and coordination of rehabilitation services. This position requires strong clinical judgment, leadership, organization, documentation skills, and the ability to work effectively with participants, staff, behavioral health providers, and community partners.
ESSENTIAL DUTIES & RESPONSIBILITIES
Rehabilitation Planning & Participant Services Complete and/or oversee initial rehabilitation assessments for newly admitted participants. Develop, review, and update Individual Rehabilitation Plans (IRPs) in accordance with required timeframes and participant needs. Complete and/or review DLA-20 assessments as required. Ensure rehabilitation goals, objectives, interventions, assessments, and progress documentation are individualized and consistent with identified participant needs. Review participant progress toward rehabilitation goals and recommend modifications when appropriate. Ensure rehabilitation activities are directed toward the development, restoration, and maintenance of skills necessary for successful community living. Promote person-centered, recovery-oriented, culturally responsive, and age-appropriate rehabilitation services. Participate in intake, transition, transfer, and discharge planning as appropriate. Collaborate with participants, behavioral health providers, and other members of the participant's care team as needed. Documentation & Medical Record Oversight Review and monitor participant medical records for completeness, accuracy, timeliness, quality, and compliance. Review rehabilitation contact notes, monthly progress summaries, rehabilitation assessments, IRPs, DLA-20 assessments, and other required program documentation. Ensure documentation supports the participant's identified rehabilitation needs, goals, interventions, and progress. Identify documentation deficiencies and provide timely guidance and corrective feedback to staff. Complete documentation assigned to the Rehabilitation Specialist within required program and regulatory timeframes. Assist with documentation required for initial and continued authorization of PRP services. Maintain confidentiality of participant information in accordance with HIPAA and organizational requirements. Staff Supervision, Education & Support Provide regular supervision, consultation, education, and rehabilitative guidance to Psychiatric Rehabilitation Coordinators/Counselors and other assigned direct-care staff. Review direct-care staff documentation and service delivery for quality and compliance. Educate staff regarding implementation of participants' IRP goals and appropriate rehabilitation interventions. Provide coaching and feedback to improve documentation quality, service delivery, and participant outcomes. Monitor staff performance as it relates to rehabilitation services and communicate identified concerns to program leadership. Participate in individual staff supervision, case consultations, team meetings, and other required program meetings. Observe staff and participant interactions when appropriate. Assist with orientation and training of new rehabilitation staff. Identify staff training needs and participate in the development or delivery of training as appropriate. Program Oversight & Quality Assurance Assist in overseeing the daily provision of psychiatric rehabilitation services. Monitor service delivery to ensure consistency with participant needs and established rehabilitation plans. Participate in internal chart reviews, quality assurance activities, compliance monitoring, and corrective-action initiatives. Assist with preparation for Maryland Department of Health, Medicaid, accreditation, payer, and other regulatory audits or surveys. Maintain availability and participate in audits, surveys, reviews, and corrective-action activities as required. Collaborate with program leadership to identify opportunities for program improvement. Report significant documentation, safety, service-delivery, or compliance concerns to appropriate leadership. Participate in incident review and follow-up when applicable. Community & Care Coordination Conduct face-to-face participant assessments and other required interactions as appropriate. Travel to participant homes and approved community locations throughout Baltimore City and Baltimore County when required. Collaborate with therapists, psychiatrists, primary care providers, case managers, community organizations, and other providers involved in participant care. Assist participants and staff with identifying appropriate community resources and supports. Develop and maintain professional relationships with community organizations and referral sources. Participate in appropriate community outreach and referral-development activities as assigned. Training & Professional Development Participate in required staff meetings, case consultations, in-service trainings, and professional development activities. Complete all onboarding, annual, regulatory, payer, accreditation, and organizational training requirements. Maintain all licenses, credentials, certifications, and continuing education required for the position. Maintain current knowledge of applicable psychiatric rehabilitation requirements and best practices. Additional Responsibilities Recognize and appropriately respond to participant emergencies and crisis situations in accordance with organizational policies and the scope of the position. Maintain professional boundaries and ethical standards. Demonstrate dependable communication and responsiveness during established working hours. Perform other duties reasonably related to the Rehabilitation Specialist role and program operations as assigned.
WORK SCHEDULE & LOCATION
This is a full-time position requiring 40 regularly scheduled hours per week, Monday through Friday . The Rehabilitation Specialist is expected to maintain the in-person availability necessary to support program participants, rehabilitation staff, and program operations in accordance with applicable Maryland regulatory and program requirements. Work responsibilities may be performed at The Crossroads' program location in Owings Mills and in approved community-based settings throughout Baltimore City and Baltimore County, including participant homes and other locations where authorized program responsibilities occur. Because psychiatric rehabilitation services are community-based, the position requires the ability and willingness to travel between the program location and community settings as needed. This position is not a fully remote position. Certain administrative responsibilities may be approved for remote completion when appropriate and consistent with program needs, leadership approval, and applicable requirements. Any remote-work component does not constitute a guaranteed remote or permanently fixed hybrid work arrangement. The Rehabilitation Specialist is expected to maintain the established work schedule and remain available to program leadership, staff, and participants during assigned working hours. Outside employment or other professional activities must not interfere with the employee's established work schedule, availability, performance, assigned responsibilities, or applicable regulatory requirements.
REQUIRED QUALIFICATIONS
Candidates must meet the applicable Maryland qualifications to serve as a Rehabilitation Specialist in a Psychiatric Rehabilitation Program. Qualifying credentials may include an individual who meets an applicable regulatory pathway as a: Maryland-licensed mental health professional; Qualified rehabilitation counselor; Certified Psychiatric Rehabilitation Practitioner (CPRP); Qualified therapeutic recreation specialist; or Other professional meeting the applicable Maryland regulatory requirements for the Rehabilitation Specialist position. Applicants are responsible for maintaining all licenses, certifications, registrations, or professional credentials required for their qualifying pathway. Additional Requirements Must be at least 21 years of age. Must be legally authorized to work in the United States. Must possess a valid driver's license. Must have reliable transportation and current automobile insurance. Must be willing and able to use a personal vehicle for authorized community-based responsibilities. Must successfully complete required background and exclusion screenings. Must obtain and maintain CPR and First Aid certification as required. Must complete all required staff trainings and certifications. Must be able to recognize emergencies and follow established emergency procedures. Must be able to communicate effectively and professionally with participants, staff, providers, and community partners. Must possess strong written communication and clinical documentation skills. Must demonstrate strong organizational, analytical, problem-solving, and time-management skills. Must be able to maintain accurate and timely electronic records. Must be comfortable utilizing electronic health records, email, Microsoft Office applications, web-based platforms, and other electronic systems required for the position. Must be able to reliably travel throughout Baltimore City and Baltimore County as required by the position.
PREFERRED QUALIFICATIONS & EXPERIENCE
Previous experience working in a Maryland Psychiatric Rehabilitation Program (PRP). Experience developing and reviewing Individual Rehabilitation Plans (IRPs). Experience completing or reviewing DLA-20 assessments. Experience reviewing psychiatric rehabilitation progress notes and monthly summaries. Experience supervising, educating, or providing rehabilitative guidance to direct-care PRP staff. Knowledge of Maryland Medicaid PRP requirements. Knowledge of COMAR requirements applicable to psychiatric rehabilitation programs. Familiarity with authorization and continued-stay documentation. Experience with accreditation standards and regulatory surveys. Experience with quality assurance, chart auditing, or compliance monitoring. Strong understanding of person-centered, recovery-oriented psychiatric rehabilitation.
