Job Description

The Rehabilitation Specialist's goal is to provide consumers living with serious and persistent mental illness a higher quality of life in a safe and therapeutic environment. The Rehabilitation Specialist will be expected to work with a diverse population and must be sensitive to the culture, religion, language, age, gender, and sexual orientation of the population served.
ESSENTIAL DUTIES 1
) Supports each resident's individualized treatment plan to assist him or her in developing the skills needed to successfully live and work more independently in the community. 2) Precisely monitors and documents consumer medication compliance. Educates consumers regarding the role and use of medications. 3) Develop therapeutic and effective working relationships with consumers, while maintaining professional boundaries. 4) Spends a majority of time in the house milieu, motivating, teaching, and assisting consumers in self-care, practical living, leisure, and interpersonal skills, as appropriate. 5) Monitors consumers' health care needs and substance abuse issues and reports concerns or incidents to consumer's primary case manager, agency RN, or supervisory staff. 6) Effectively intervenes in consumer crises, including consulting with agency staff, on-call supervisors, and Designated Mental Health Professionals regarding potentially dangerous or problematic situations. 7) Encourages and assists consumers to engage in work or other meaningful activities. 8) Reminds consumers of appointments with prescriber and assists in preparing them for other appointments, as necessary. 9.) Documents each consumer's symptoms and behavior in the daily shift log, and in the client's record as appropriate. 10.) Conduct regular safety checks of the facility. Monitor and document the presence of residents on site. 11.) Other duties as assigned including, but not limited to: shredding confidential documents, copying and stocking forms and med room supplies, and general cleaning of the staff office space. 12.) Navigate two flights of stairs and lift up to 30 pounds. 13.) Must be able to work on-site, remote work is not available for this position.
RELATIONSHIP WITH COMMUNITY AND CO-WORKERS 1
) Contributes to cooperative working relationships with clinical teammates and with supervisors, and other staff within the agency. 2) Solicits and provides case consultation and coordination as needed. 3) Presents a positive professional image of the agency to referral sources and other service providers and community resources needed by agency consumers.

MINIMUM QUALIFICATIONS
High school diploma or equivalent required. : Bachelor's degree in Human Services or a related field, with experience working with individuals with serious and persistent mental illness (SPMI). Other residential, behavioral health, or social service experience may substitute for the preferred educational qualifications. Valid driver's license and an acceptable driving record required, as the position involves operating Agency vehicles. Criminal background check and fingerprint-based identification check, required for working with vulnerable adults.
REQUIREMENTS TO BE COMPLETED AFTER HIRE
The following requirements are not required at the time of hire but are required to work in this position. The Agency will provide required training and will compensate employees for time spent completing Agency-required training and certification requirements. Agency Affiliated Counselor (AAC) registration, as required by the state. Home Care Aide (HCA) certification, including Agency-provided training. First Aid/CPR certification, including Agency-provided training. TB blood test. Any additional training or certifications required by the Agency or applicable state regulations.

infection prevention guidelines to control the spread of COVID-19 and other acute viral respiratory infections. Use of Personal Protective Equipment (PPE): Correctly wear required face coverings, masks, or respirators (such as N95s) based on task risk assessments, resident health status, or specific transmission-based precautions.
WAGES & HOURS
This is a 40 hour per week, non-exempt position.
Schedule:
Tuesday, Thursday, Friday and Saturday, 1:30pm to 11:30pm
Rate of Pay:
$34.40 per hour with an additional $2.50 per hour for weekend shifts. Counseling staff are encouraged to participate in the agency's on-call system. On-call hours involve being reachable by phone and available to report to work immediately if needed. These hours are compensated at $3.00 per hour while off-site, with an additional $200 paid if called in. Flexibility in covering overnight, weekend, or holiday shifts is also encouraged when coverage is needed.
