Job Description

Connected Collaborations is actively seeking a passionate Certified Occupational Therapy Assistant (COTA) to join our growing team serving San Tan Valley, Casa Grande and Maricopa, Arizona . We currently have an established caseload available in these areas , allowing for a seamless transition into the role. Once onboarding is complete, clinicians can begin seeing patients immediately and start building their schedule right away. CompensationCOTA Pay Starting at $90 per hour for ongoing therapy services Compensation may increase based on service area, experience, and caseload availability

Meaningful Work Make a direct impact in the lives of individuals receiving services in home health and group home settings while working alongside a collaborative interdisciplinary team. Essential Responsibilities Evaluate and treat patients with a variety of occupational therapy needs under the direction of an Occupational Therapist Implement individualized treatment plans and therapeutic interventions Provide functional and developmental support to improve patients' independence and quality of life Complete SOAP notes, progress reports, and other required documentation through our user-friendly EMR system Collaborate with caregivers, families, and interdisciplinary team members Provide services to a diverse caseload including individuals with: Autism Spectrum Disorder Down Syndrome Cerebral Palsy Cognitive Disabilities Chromosomal Syndromes Developmental and neurological conditions Maintain accurate and timely documentation Work independently while maintaining strong communication with supervising occupational therapists and the Connected Collaborations team Qualifications Completion of an accredited Certified Occupational Therapy Assistant (COTA) program Active Arizona COTA license or eligibility by start date Ability to provide services in a home health/community-based setting Pediatric experience preferred Additional Requirements Current CPR Certification Valid Arizona Fingerprint Clearance Card Article 9 Training or willingness to complete upon hire with company support Reliable transportation for community-based visits About Connected Collaborations Connected Collaborations is a clinician-owned and operated healthcare company dedicated to providing exceptional, coordinated care in home health and group home settings.
