Park Avenue Health & Rehabilitation Center is a 200 bed skilled nursing facility with a 5-star rating from CMS and an excellent reputation within our community. The Occupational Therapy Assistant is responsible for assisting the Occupational Therapist by providing rehabilitative therapy to patients upon completion of the evaluation and plan of care following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services. This position reports to the Director of Rehabilitation Services and is supervised by an Occupational Therapist.
Qualifications:
Must possess current/active Arizona COTA Licensure (required) Must possess or be eligible to obtain AZ Fingerprint Clearance Card (required) One year experience in long-term care or related setting (preferred)Our Values :
