Job Description
South Pacific Rehab Services seeks an Occupational Therapist Assistant to provide resident-centered care in skilled nursing and post-acute settings. Under the supervision of an Occupational Therapist, you will deliver individualized treatments that promote functional recovery, independence in ADLs, safety, and quality of life. Responsibilities include implementing treatment plans, documenting in the EMR, collaborating with an interdisciplinary team, and educating residents and families. Responsibilities Deliver high-quality occupational therapy services under the supervision of an Occupational Therapist in skilled nursing and post-acute settings Implement individualized treatment plans focused on functional recovery, ADLs, and quality of life Document evaluations, daily notes, and progress in the EMR accurately and timely Collaborate with an interdisciplinary team to support resident-centered, ethical care Educate residents and families on home programs, safety, and functional strategies Participate in care conferences and contribute clinical input to discharge planning Follow all regulatory, ethical, and company standards for clinical practice Engage in ongoing education and training supported by clinical leadership Required Skills Occupational therapy interventions Treatment planning and progression Functional mobility and ADL training Therapeutic exercise and activities Cognitive and safety retraining Documentation and EMR proficiency Interdisciplinary team collaboration Patient and family education SNF/post-acute rehab knowledge Infection control and safety compliance
