Skilled Nursing Facility (SNF) Join Our Therapy Team at Garden City Health Care Center Garden City Health Care Center is seeking a compassionate and skilled Certified Occupational Therapy Assistant (COTA) to join our dedicated rehabilitation team. General Purpose The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist with patient-related activities and provides direct patient care in accordance with physician orders, treatment plans, and applicable regulations. Essential Duties and Responsibilities Treat residents as directed by the supervising Occupational Therapist Implement treatment plans according to physician orders Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility policy, and payer requirements Communicate resident progress, problems, and plans with the supervising OT and interdisciplinary team Assist nursing staff with training of Restorative Aides as appropriate Participate in in-service training programs and care conferences Educate residents, families, and caregivers on maintenance programs and discharge planning Participate in discharge planning and transition of care Maintain a clean, safe, and organized treatment area Report equipment issues to ensure safe and effective operation Maintain positive, professional relationships with residents, families, and staff Supervisory Responsibilities This position has no supervisory responsibilities Qualifications Education & Experience Graduate of an accredited Occupational Therapy Assistant program Skilled nursing or geriatric experience preferred Licensure & Certifications Current, active California Occupational Therapy Assistant (COTA) license Must meet all continuing education requirements Must maintain license in good standing with the State Board Skills Proficient in electronic documentation systems ( Casamba and PointClickCare preferred ) Strong communication, organization, and problem-solving skills Ability to work independently and as part of a collaborative rehab team Physical Demands Frequent standing, walking, reaching, pushing, and pulling Ability to exert up to 50 pounds frequently and 100 pounds occasionally Occasional climbing, balancing, stooping, kneeling, crouching, or crawling Visual abilities include close, distance, color, peripheral, and depth perception Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions Work Environment Skilled nursing and rehabilitation environment Noise level typically low to moderate Reasonable accommodations available Additional Information This job description does not restrict management's right to assign or reassign duties and responsibilities at any time.
