The Occupational Therapy Assistant will provide skilled occupational therapy interventions under the direction and supervision of a licensed Occupational Therapist (OT) to help patients improve functional independence, safety, participation in activities of daily living, and quality of life. This is an independent contractor (1099) opportunity offering flexible scheduling and competitive compensation based on experience, qualifications, and assignment. The OTA is responsible for maintaining all requirements applicable to independent contractor status, including required licensure, professional liability insurance, taxes, transportation, and scheduling availability. Essential Duties and Responsibilities Provide occupational therapy treatment services in patients' homes under the direction and supervision of a licensed Occupational Therapist; Implement the occupational therapy treatment plan established by the supervising OT; Provide therapeutic activities and interventions designed to improve functional independence; Assist patients with activities of daily living (ADLs), including dressing, bathing, grooming, toileting, feeding, and other functional activities as appropriate; Provide interventions to improve upper-extremity strength, coordination, range of motion, and functional use; Provide fine motor and functional coordination activities; Assist with interventions addressing cognitive, perceptual, and sensory-motor skills as directed by the supervising OT; Provide home safety and fall prevention interventions; Assist patients with safe performance of functional activities within their home environment; Reinforce prescribed therapeutic activities and home programs established by the supervising OT; Educate patients and caregivers regarding prescribed treatment activities, safety techniques, adaptive strategies, and energy conservation; Assist with training in the appropriate use of adaptive equipment and durable medical equipment as directed by the OT; Assist with recommendations and implementation of environmental modifications as directed by the supervising OT; Monitor patient tolerance and response to treatment; Observe and report changes in patient condition, functional status, safety, cognition, or ability to participate in treatment to the supervising OT; Communicate patient progress, concerns, and barriers to treatment to the supervising Occupational Therapist; Coordinate care with nurses, physicians, physical therapists, speech therapists, caregivers, and other members of the interdisciplinary team as appropriate; Participate in care conferences and care coordination activities when requested; Complete accurate, timely, and clinically appropriate documentation for all services provided; Maintain compliance with Medicare, CMS, HIPAA, California requirements, payer requirements, and Nurse Next Door policies and procedures; Maintain patient confidentiality and protect patient health information; Perform other duties within the OTA's scope of practice as reasonably assigned. Important Scope-of-Practice Requirement The OTA will work under the direction and supervision of a licensed Occupational Therapist. The OTA does not independently perform occupational therapy evaluations, establish the occupational therapy plan of care, independently diagnose patients, or independently make changes to the established plan of care . All services must be provided within the OTA's education, training, competency, licensure, and applicable California laws and regulations. Qualifications Graduate of an accredited Occupational Therapy Assistant program; Current and unrestricted California Occupational Therapy Assistant license ; Current CPR/BLS certification; Minimum one (1) year of clinical experience preferred; Home health experience strongly preferred; Experience with geriatric, medically complex, rehabilitation, or home-based populations preferred; Knowledge of ADL training, functional mobility, therapeutic activities, upper-extremity function, fine motor coordination, home safety, fall prevention, and adaptive equipment; Strong ability to recognize and report changes in patient condition; Excellent communication, interpersonal, documentation, and organizational skills; Ability to work independently in patients' homes while maintaining appropriate communication with the supervising OT; Valid California driver's license; Reliable transportation and proof of current auto insurance; Ability to travel throughout the assigned service area. Independent Contractor Requirements As a 1099 independent contractor, the OTA is responsible for: Maintaining an active California OTA license; Maintaining required professional certifications; Maintaining professional liability/malpractice insurance; Maintaining appropriate automobile insurance; Providing reliable transportation; Providing a cell phone and basic equipment necessary to perform assigned duties; Complying with all applicable tax and independent contractor obligations; Maintaining all required credentials and documentation. Documentation Requirements All clinical documentation must be completed accurately, timely, and in accordance with Nurse Next Door policies, Medicare/CMS requirements, payer requirements, and applicable federal and California regulations. Documentation must be submitted through the designated electronic health record system. Timely completion of clinical documentation is an essential requirement of providing services for Nurse Next Door. Physical Requirements The OTA must be able to: Travel between patient homes; Assist patients with functional activities and mobility; Stand and walk for extended periods during patient visits; Bend, kneel, squat, reach, and lift as necessary to provide patient care; Demonstrate therapeutic activities and functional techniques; Safely perform required patient-care activities within the home environment.
