Geriatric Rehabilitation Team:

In-house rehabilitation What You'll Do Provide high-quality occupational therapy services under the supervision of a licensed Occupational Therapist. Help residents improve independence with activities of daily living (ADLs) and functional mobility. Implement individualized treatment plans based on resident needs and goals. Provide therapeutic interventions designed to improve strength, coordination, function, and independence. Monitor and document resident progress and communicate changes to the supervising therapist and care team. Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to support resident-centered care. Encourage residents throughout the rehabilitation process and promote dignity, safety, and quality of life.
