Minimal Office Visits Supportive Clinical Team Electronic Documentation Growing, Award-Winning Agency Your Role:
Conduct patient assessments and assist with ADLs (bathing, dressing, grooming, mobility). Develop individualized care plans and provide hands-on therapy interventions. Educate patients and caregivers on exercises, safety, and adaptive equipment. Collaborate with physicians, nurses, PTs, and social workers. Document progress and assist with discharge planning.
Qualifications:
Licensed PT, OT, PTA or OTA 1+ year of Home Health experience (required) Strong knowledge of anatomy, physiology, and medical terminology Experience with Kinnser EMR preferred Compassionate, independent, and collaborative Pay Range (DOE
  • Depending on Experience): Physical Therapist (PT) / Occupational Therapist (OT): $68
  • 75/hour Physical Therapist Assistant (PTA) / Occupational Therapist Assistant (OTA): $58
  • 64/hour
Work Location:
On the road (patients' homes in Santa Clarita Valley)
