Community Health and Rehab is a 101 bed facility with a state of the art therapy gym! We are looking for a Graduate of an accredited occupational therapy program, one year of experience in a long term care facility is preferred but not a deal breaker. Essential Job duties include, but are not limited to Assessing Residents functional levels and capabilities Follows resident care plan and provides OT treatments under the direction of a licensed OT Makes Recommendations related to resident therapy treatments Participates in discharge planning processes Thoroughly documents treatment progress in resident charts in a accurate and timely manner Completes required facility forms and documents to ensure compliance with state and federal regulations Provides high quality customer service to residents and their family members Provides training and education to family residents and staff for occupational therapy Assists with transporting residents to and from therapy Attends meetings and communicates information in a productive fashion

$30.00 - $35.00 per hour
