Renewal Rehab seeks a compassionate Certified Occupational Therapy Assistant to support seniors in skilled nursing and post-acute care settings. In this on-site role, you will deliver evidence-based interventions to enhance functional independence, safety, and quality of life. Working closely with Occupational Therapists and an interdisciplinary team, you will implement treatment plans, complete timely documentation, and educate patients and families. Responsibilities Implement occupational therapy treatment plans under the supervision of an Occupational Therapist to improve seniors' functional independence. Conduct therapeutic activities focusing on ADLs, mobility, strength, balance, and safety in skilled nursing and post-acute settings. Accurately document treatments, progress notes, and outcomes in accordance with regulatory and facility standards. Collaborate with OTs, nursing, and interdisciplinary teams to support patient-centered care plans and discharge goals. Educate patients and families on home programs, safety techniques, and use of adaptive equipment.
