Job Description

Renewal Rehab seeks a compassionate Certified Occupational Therapy Assistant to support seniors in skilled nursing and post-acute care settings. In this on-site role, you will deliver evidence-based interventions to enhance functional independence, safety, and quality of life. Working closely with Occupational Therapists and an interdisciplinary team, you will implement treatment plans, complete timely documentation, and educate patients and families. Renewal Rehab offers a collaborative, patient-centered culture, strong clinical support, and opportunities for professional growth while maintaining manageable caseloads and a focus on meaningful outcomes for older adults. Responsibilities Implement occupational therapy treatment plans under the supervision of an Occupational Therapist to improve seniors' functional independence. Conduct therapeutic activities focusing on ADLs, mobility, strength, balance, and safety in skilled nursing and post-acute settings. Accurately document treatments, progress notes, and outcomes in accordance with regulatory and facility standards. Collaborate with OTs, nursing, and interdisciplinary teams to support patient-centered care plans and discharge goals. Educate patients and families on home programs, safety techniques, and use of adaptive equipment. Contribute to a positive, respectful, and integrity-driven culture aligned with Renewal Rehab's mission and values. Required Skills Occupational therapy treatment planning Geriatric rehabilitation ADL and functional mobility training Skilled nursing/post-acute care documentation Use of therapy modalities and adaptive equipment Interdisciplinary team collaboration Care planning and family education Outcome measurement and functional assessments Time management and caseload prioritization Electronic health record (EHR) proficiency
