Your Key Responsibilities:

Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life. Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs). Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards. Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence. Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery.. Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.
