Job Description
St. John's Health Care P.C. Medicare Division is seeking a compassionate and dedicated Certified Occupational Therapist Assistant to join our team of home health providers in Ludington, MI, and surrounding areas. If you value work-life balance, home health care might be the perfect fit for you!
Your Role:
- Part-time and PRN positions available, with the opportunity to build a full-time caseload.
- Provide physician-ordered Occupational Therapy to patients in their homes.
- Perform visits
- Educate patients and caregivers/families on Home Exercise Programs (HEPs).
- Collaborate with other health care professionals within the interdisciplinary team.
- Consult with physicians regarding patient changes and progress.

Our goal is to provide quality care in the comfort of patients' homes, serving homebound patients in Manistee, Mason, Lake, Osceola, and Oceana counties.We Are Seeking:
- A current Certified Occupational Therapist Assistant licensed in the State of Michigan.
- CPR certification and TB test at the time of hire.
- At least 1 year of experience in home health care or a facility (preferred but not required).
- The ability to meet the physical demands of the job.
- Strong communication skills and dedication to quality patient care.
