Job Description

St. John's Health Care P.C. Medicare Division is seeking a compassionate and dedicated Certified Occupational Therapist Assistant to join our team of home health providers in Ludington, MI, and surrounding areas. If you value work-life balance, home health care might be the perfect fit for you!
Your Role:
  • Part-time and PRN positions available, with the opportunity to build a full-time caseload.
  • Provide physician-ordered Occupational Therapy to patients in their homes.
  • Perform visits
  • Educate patients and caregivers/families on Home Exercise Programs (HEPs).
  • Collaborate with other health care professionals within the interdisciplinary team.
  • Consult with physicians regarding patient changes and progress.
We Serve:
Our goal is to provide quality care in the comfort of patients' homes, serving homebound patients in Manistee, Mason, Lake, Osceola, and Oceana counties.
We Are Seeking:
  • A current Certified Occupational Therapist Assistant licensed in the State of Michigan.
  • CPR certification and TB test at the time of hire.
  • At least 1 year of experience in home health care or a facility (preferred but not required).
  • The ability to meet the physical demands of the job.
  • Strong communication skills and dedication to quality patient care.
