Job Description We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team within a Skilled Nursing Facility in Minnesota. Working under the supervision of a licensed Occupational Therapist, the COTA will provide skilled occupational therapy services designed to help residents improve functional independence, safety, and overall quality of life. This position is ideal for a clinician who enjoys working with the geriatric population and thrives in a collaborative, interdisciplinary care environment. Responsibilities Implement occupational therapy treatment plans established by the supervising Occupational Therapist. Assist residents with therapeutic activities and interventions designed to improve activities of daily living (ADLs), mobility, coordination, strength, endurance, and safety awareness. Monitor resident responses to treatment and communicate progress, concerns, and changes in condition to the Occupational Therapist. Document treatment sessions, patient progress, and outcomes in accordance with facility, state, federal, and payer requirements. Collaborate effectively with Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, residents, and family members. Participate in interdisciplinary care conferences and discharge planning activities. Maintain a safe, organized, and effective treatment environment. Follow all facility policies, professional standards, and ethical guidelines. Qualifications Graduate of an Accreditation Council for Occupational Therapy Education (ACOTE)-accredited Occupational Therapy Assistant program . Current Minnesota Occupational Therapy Assistant License issued by the Minnesota Board of Occupational Therapy Practice, or eligibility to obtain licensure prior to start date. Current certification through the National Board for Certification in Occupational Therapy (NBCOT) or eligibility as required for Minnesota licensure. Skilled Nursing Facility, rehabilitation, or geriatric experience preferred but not required. Strong communication, interpersonal, organizational, and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary healthcare team. Compassionate, patient-centered approach to care. New graduates are encouraged to apply.
