Overview Nina's Health Care Fairfax is seeking compassionate, dependable, and experienced Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide skilled home health services to patients throughout Northern Virginia and surrounding service areas. This is a great opportunity for therapists looking for a flexible home health schedule and the ability to work independently while helping patients improve their safety, independence, and ability to perform activities of daily living in their homes. NOT facility based. Skilled visits in patient's homes. Responsibilities Perform initial occupational therapy evaluations and reassessments as ordered. Develop individualized plans of care based on the patient's functional needs and physician/provider orders. Assess activities of daily living, functional mobility, home safety, strength, coordination, endurance, and need for adaptive equipment. Provide skilled occupational therapy treatment to improve the patient's independence and safety in the home. Establish measurable goals and update the treatment plan based on the patient's progress. Provide patient and caregiver education regarding safety, ADLs, energy conservation, adaptive techniques, and home exercise programs. Communicate changes in condition and treatment recommendations to the physician and interdisciplinary home health team. Complete all required clinical documentation accurately and within agency timeframes. Supervise COTA services when applicable and in accordance with Virginia requirements and agency policy. Participate in care coordination, discharge planning, and case conferences as needed. Certified Occupational Therapy Assistant (COTA)

Responsibilities:

Provide occupational therapy treatment according to the established OT plan of care. Assist patients with improving independence in activities of daily living and functional activities. Provide therapeutic activities, exercises, adaptive techniques, and patient/caregiver education as directed by the supervising OT. Observe and report the patient's response to treatment and any significant changes in condition to the supervising OT and agency. Document each home health visit accurately and timely. Coordinate with the supervising Occupational Therapist and interdisciplinary care team. Follow all physician orders, agency policies, infection-control practices, and home health requirements.

Requirements :

Current and unrestricted Virginia Occupational Therapist license for OT applicants OR appropriate Virginia COTA licensure/certification for COTA applicants. Graduate of an accredited Occupational Therapy or Occupational Therapy Assistant program. Current CPR/BLS certification. Current TB screening/testing as required. Ability to successfully complete required background checks and employment screening. Reliable transportation and ability to travel to patients' homes. Valid driver's license and automobile insurance. Ability to independently manage a home health caseload and schedule. Strong communication, organizational, and documentation skills. Ability to complete electronic medical record documentation. Home health experience is preferred. Medicare-certified home health and OASIS experience is strongly preferred for Occupational Therapists.
