Confirm effective dates and plan status. Check deductible, annual maximum, and remaining benefits. Verify coverage percentages for diagnostic, preventive, restorative, periodontal, endodontic, oral surgery, and prosthodontic services. Check frequency limitations and waiting periods. Verify exclusions, downgrades, missing-tooth clauses, and age limitations. Confirm whether preauthorization or predetermination is required. Check in-network vs. out-of-network benefits and fee schedules. Document insurance verification reference number, representative, date, and details. Review clinical findings, X-rays, perio charting, and existing treatment. Identify urgent/active disease and prioritize treatment. Develop recommended treatment plan and alternatives. Assign appropriate CDT codes and estimated fees. Calculate estimated insurance payment and patient portion. Submit predetermination/preauthorization when appropriate. Review treatment plan and financial estimate with patient. Obtain informed consent and schedule treatment. Document all insurance and treatment-planning discussions in the patient record.
