• 6 exam rooms total
• 1 procedure room
• 2 isolation rooms
• 1 pediatric room
• 2 standard exam rooms
• Patient volume: 20-40 patients per day
• Support staff: 2-3 per shift (mix of RNs and CMAs)
• Medical Director available by phone, email, and Epic during the day
• Patient population: All ages

• Requirements:

• Active Oregon license
• Required
• Oregon DEA
• Preferred
• BLS
• Required
• Post-graduate experience
• Required (must work independently)
