Community Bridges, Inc. (CBI) is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. Job Summary The Wellness Navigator will help to remove personal and environmental obstacles to health care access. The Wellness Navigator is responsible for working closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy, and support primarily in home and/or in the community. The Wellness Navigator will engage, educate, and offer support to individuals, their family patients, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.
