Gellert Health is expanding! We are opening doors in Tucson and we are looking for the right person to help us in taking this next step! The Health Navigator - Team Lead is a working leadership role responsible for both direct patient navigation and oversight of the Tucson Navigator team. This individual carries an active caseload while providing supervision, quality assurance, and team development as the team grows. Initially this position will have one direct report over the first year we would like to grow the Tucson team to 5 Navigators. The role ensures patients who may be experiencing homelessness, serious mental illness, and complex medical conditions, receive coordinated, compassionate, and compliant care, and serves as the primary point of leadership for Navigation staff in Tucson. Essential Duties and Responsibilities Team Leadership

• Lead, mentor, and support Navigators as the Tucson team grows
• Maintain working knowledge of each Navigator's caseload; assign and review cases based on complexity and workload
• Provide regular supervision, guidance, and performance feedback
• Address performance concerns and support staff professional development
• Work with HR in identifying training needs and provide ongoing training for new hires Navigation (Individual Caseload)
• Coordinate patient appointments, transportation, and care logistics, including direct transport of patients as needed
• Maintain organization of patient medications, providers, and care plans
• Serve as primary point of contact between patients and the healthcare system
• Identify and address social, safety, and health-related barriers to care
• Document all patient interactions and services accurately and timely in the HER
• Use personal vehicle to transport patients to and from appointments (auto stipend provided) Quality Assurance & Compliance
• Work with Quality and Compliance Manager in conducting chart audits and case reviews
• Monitor EHR documentation for accuracy and completeness
• Ensure compliance with Gellert Health policies and applicable regulatory requirements Patient Advocacy
• Support Navigators in developing effective patient care plans
• Intervene directly in escalated or complex patient situations
• Ensure patients receive timely, coordinated services Collaboration & Community Relations
• Represent Gellert Health with Tucson-area referral partners and community organizations
• Coordinate care across healthcare providers and community resources
• Report on team performance and office operations to leadership Qualifications
• Experience in healthcare, social work, or community support; prior navigator or case management experience preferred
• Leadership experience in case management or a related field
• Associate's or Bachelor's degree, or equivalent work experience
• Experience working with culturally diverse or vulnerable populations
• Must use personal vehicle to transport patients; valid Arizona driver's license, clean motor vehicle record, reliable four-door vehicle, and auto insurance required
• CPR/First Aid certification
• Ability to pass background check and drug screening
• Spanish bilingual a plus, not required Working Conditions
• Full-time, Monday-Friday
• In-person/in-field position based in Tucson, AZ
• Requires regular local travel, in personal vehicle, transporting patients

From $52,000.00 per year
