The position requires the HCN to ensure Veterans access physical and mental health services, supportive services and family members are educated on the issues the Veteran is experiencing.

Preferred:

Master's Degree in Social Work or Rehabilitation and 2 years of direct experience working with Veterans. Minimum Bachelor's Degree in Social Work or Rehabilitation and 3 years' direct experience working with Veterans. The salary range is $58,698

• $71,347, depending on education and experience, plus benefits.
