Serve as a non-clinical liaison and coach for patients accessing substance use treatment. Conduct outreach, reduce barriers to care, assist with appointment scheduling in Epic, and connect individuals to vital healthcare and social services.
Key Responsibilities:
Conduct outreach to referred patients and engage individuals seeking substance use care and harm reduction support. Schedule and book patient appointments using Epic Hyperspace. Identify and eliminate barriers to care by connecting patients to insurance, primary care, mental health, and social services. Collaborate with counselors and clinical teams to support treatment plans and maintain thorough documentation. Provide empathetic, trauma-informed guidance to enhance patient satisfaction and engagement.
Requirements:
Proven experience working with underserved communities and individuals facing substance use disorders. Proficiency in patient scheduling and booking using Epic Hyperspace. Experience in front-desk administration or healthcare office workflows. strong alignment with trauma-informed, harm-reduction, and strengths-based care principles. Full medical clearance and health screening completion required per institutional policy.
