Baptist Health is hiring a Resource Navigator to join The Players Center in Jacksonville, FL. This is a full-time opportunity. The Resource Navigator supports patients and families by identifying social factors that may impact their health and well-being. This role helps connect families with appropriate community resources and provides follow-up to ensure connections are made and resources are beneficial.
Resource Navigator Job Responsibilities:
Screen patients and families for social determinants of health Identify opportunities to improve health and address barriers to care Connect patients and families with appropriate community resources and support services Provide information and assistance to help families access available resources Follow up with patients and families to confirm connections to resources were successfully made Assess whether identified resources are meeting the needs of patients and families Document resource navigation activities and follow-up appropriately
Resource Navigator Experience:
Less than 1 year Community Service Experience (required) Less than 1 year Computer Application Experience (required) Experience working with patients, families, or community organizations (preferred) Experience connecting individuals with community resources (preferred)
Resource Navigator Education & Credential Requirements:
