ABOUT WELL&YOU RURALREACH
Well&You RuralReach will open four permanent satellite clinics across Northeast Florida, including rural Baker and Nassau counties, bringing chronic disease management, vascular and limb-salvage care, cardiac monitoring, women's health, and non-opioid pain care to residents who now drive 35 to 40 miles for specialty care. In communities where heart disease, diabetes, stroke, and amputation rates run well above Florida's average, this award turns Well&You's long-standing local roots and relationships into permanent access points for care. Staffed by board-certified physicians and APRNs, these clinics will build sustainable rural health infrastructure and bring a better way to care to rural Northeast Florida.
Job Summary:
The Patient Navigator is the patient's first point of contact and the person who keeps a rural patient's care from falling apart between visits. This role handles intake, eligibility and benefits verification, scheduling, and follow-up for patients traveling long distances for care — sequencing imaging, consultation, and procedures into as few trips as possible, and resolving the transportation, coverage, and connectivity barriers that cause rural patients to no-show or drop out of care entirely. Navigators supporting rural sites work with a high degree of independence: often the only administrative presence on site, frequently the sole point of contact for a patient across several weeks of care, and expected to resolve problems in the moment rather than route them elsewhere. The role requires an experienced medical-office professional with a strong foundation in insurance and scheduling logic, sound judgment about what to solve and what to escalate, and the initiative to follow a patient through to a completed visit without being prompted.
Duties/Responsibilities:
Answer all phone calls in a professional and courteous manner Meticulously intake patient information on the phone and record in EMR Greet and attend to patients during check-in, including form completion, insurance and co-pays Establish first contact with the patient and deliver a positive patient experience Capture and document patient demographic verification Capture and document insurance eligibility verification Track prior authorizations for procedures and office visits, following each one through to approval Schedule appointments with attention to the complex detail of each patient's plan of care Build rural patients' schedules to consolidate imaging, consultation, and procedures into as few trips as possible, and confirm every prerequisite is complete before the patient travels Serve as the single point of contact for assigned rural patients across their episode of care, following up proactively rather than waiting for the patient to call Identify and resolve access barriers — transportation, caregiver and work constraints, coverage gaps, device or connectivity limits, language and health literacy — using internal resources and established community partnerships Work no-show, cancellation, and unscheduled referral lists daily and reach back out to recover those patients Coordinate with rural primary care practices, rural health clinics, FQHCs, and referring offices to obtain records, close referral loops, and confirm patients arrive prepared Collection of patient payments, including explaining balances, coverage, and self-pay or sliding-scale options accurately Complete accurate documentation of patient visits Send medical records and oversee FMLA and disability paperwork completion Liaise between medical practices with discretion and professionalism Professionally assist doctors, staff, visitors, and patients Complete daily audits of assigned work and correct errors before they reach billing Capture complete and accurate demographic, barrier, and outcome data required for rural health grant reporting Travel to assigned rural clinic sites and community outreach events as scheduled
Required Skills/Abilities:
Must understand prior authorizations for procedures and office visits Strong working knowledge of insurance eligibility, benefits verification, and the rural payer mix, including Medicaid, Medicare, managed care plans, and self-pay or sliding-scale arrangements Proven ability to work independently at a remote or thinly staffed site with limited direct supervision Strong problem-solving instincts and sound judgment about what to resolve directly and what to escalate Self-starting and persistent; follows a patient through to a completed visit without being reminded Must be able to multitask effectively; critical time management skills necessary Meticulous attention to detail across high volumes of authorizations, schedules, and documentation Demonstrated ability to interact with healthcare professionals, patients, and insurances Excellent customer service skills and superior communication and interpersonal skills Knowledge of standard practices and procedures in the medical outpatient field Ability to work under pressure and react effectively to emergency situations Thrives in a fast-paced environment and possesses the poise to work under pressure Ability to be flexible yet place structure and order in a startup environment Cultural competence and ease with rural patient populations, including patients with limited health literacy or long-standing gaps in care Temperament and discipline to handle information with a high level of confidentiality Requires strong proficiency in technology Requires strong proficiency in AI and AI workflows Willingness and ability to travel to rural clinic sites and outreach events
Education and Experience:
Bachelor's degree highly preferred; Substantial directly relevant medical office experience may be considered in lieu of a degree. At least 3 full years of direct experience in a medical office, patient access, or care coordination role in a healthcare setting Demonstrated hands-on prior authorization and insurance verification experience required Experience in rural health, community health, an FQHC, or a rural health clinic preferred Demonstrated history of working independently with minimal on-site supervision Valid Florida driver's license, reliable transportation, and willingness to travel to rural sites and outreach events
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times. Regular travel between the Jacksonville area and rural clinic sites, including some early start times and occasional evening or weekend outreach events. Must be able to work productively in smaller rural facilities and temporary event spaces with limited support staff on site.
