seeks a Medical Liaison to coordinate communication between healthcare providers, patients, and internal teams. Responsibilities include supporting patient care coordination, facilitating referrals, maintaining accurate medical records, and enhancing patient understanding of treatment plans. The role collaborates closely with physicians and clinical staff to ensure quality service and timely follow-up. Responsibilities Serve as a primary liaison between physicians, clinical staff, patients, and external providers to support coordinated care. Communicate treatment plans, follow up instructions, and educational information clearly to patients and families. Coordinate referrals to specialists, diagnostics, and laboratory services and track follow up appointments and outcomes. Review medical records for completeness, accuracy, and adherence to clinical protocols. Support preventative care initiatives, including patient outreach and education on screenings, vaccinations, and chronic disease management. Collaborate with interdisciplinary teams to identify and close gaps in care and improve care pathways. Document interactions and care coordination activities accurately in the electronic health record while maintaining patient confidentiality. Assist with data collection and reporting for quality improvement and clinical audit projects. Build and maintain relationships with community partners, external healthcare providers, and insurance representatives. Provide feedback from patients and families to help enhance patient experience and service delivery.
