Job Description
Join Senior Citizens, Inc.'s Meals on Wheels Program Senior Citizens, Inc. is seeking several qualified individuals to conduct nutritional assessments for Meals on Wheels clients in the coastal counties of Camden, Glynn, and McIntosh. These are PRN positions paid on a per-assessment basis. Mileage reimbursement is not provided.
- One PRN assessor will serve Glynn and McIntosh counties.
- One PRN assessor will serve Camden County.
Responsibilities include:
- Meeting with clients to discuss their nutritional and daily living needs.
- Completing nutritional assessments when clients begin receiving services and at least annually thereafter, as required for continued meal service.
- Identifying concerns related to clients' physical, financial, emotional, or support needs.
- Recommending appropriate follow-up actions to the organization when concerns are identified.
- Completing all required assessment documentation accurately and returning the materials to the organization for retention in the client's record.
