Job Description

The QIDP is an Integral member of the leadership team for New Hope's Intermediate Care Facility for individuals with intellectual disabilities, (ICF/ID). The QIDP coordinates the service design and support for approximately 18 adults with intellectual or other Developmental Disabilities. The QIDP is responsible for the coordination of each person's total plan, within assigned caseloads, acts as the person's advocate, and as liaison for the facility and person with the family and external funding agencies. The QIDP must possess strong organizational and communication skills and be self-directed. Applicant needs to hold a bachelor's degree in a human services field or be an RN and have at least one year of experience working with people with disabilities.
