Location:
Horizons Reno County Full Time Equivalent (FTE): 1 Job position summary: Provide whole person care management services to assist clients connect with resources in an effort to improve areas related to medical care, mental health care and overall social determinants of health. This will be done through connecting consumers with community resources and ongoing coordination of care. Partner with treatment teams to identify needs a consumer may have that impact their quality of life Connect consumers with community resources and assist with coordination of housing, transportation, and any other social services to support the consumers overall wellbeing. Build community partnerships and develop coordination agreements to improve efficiencies of the coordination process between organizations Maintain collaborative communication between staff, consumers and consumer's families Assist in care coordination when referring consumers to external resources and provide follow up contact to ensure the consumer's needs were met Assists veterans in connecting with resources through Veteran Affairs and is identified as a primary behavioral health care coordinator on behalf of the veteran consumer assigned to them Data collection and tracking of performance indicators for consumers in collaboration with treatment teams Gather and provided information to treatment team relevant to treatment planning and ongoing service delivery Maintain documentation in the medical record Other duties as assigned.MINIMUM KNOWLEDGE AND SKILLS REQUIRED

High school diploma with an additional 2 or more years' experience working in a mental health setting OR Associates degreePREFERRED EDUCATION AND EXPERIENCE
Associates degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics) Experience related to care coordination in the medical or mental health field Three to five years' professional experience in mental health services or a related fieldLICENSURE OR REGISTRATION REQUIRED
Within 30 days of employment the employee must have completed CPST Case Management on-line training, KHS on-line training. Within 60 days of employment, the employee must have completed Adult Case Management training.PREFERRED LICENSURE OR REGISTRATION
