Job Description

Role Overview The Substance Use Navigator (SUN) helps patients with substance use disorders (SUDs) move from crisis to a clear plan by linking them to evidence-based treatment, recovery supports, and community resources. This role focuses on identifying patients who could benefit from services, offering brief screening and motivational encouragement, and using a prevention and risk reduction approach to assisting patients with SUD—including helping patients access Narcan and medication for addiction treatment (MAT). You will also coordinate referrals, arrange follow-up appointments, and assist with discharge planning to reduce gaps in care. Working alongside clinicians, behavioral health, social services, and community partners, the SUN helps remove barriers, strengthen continuity, and reinforce compassionate, patient-centered support.
