As a Hospital Liaison, you will serve as the primary liaison between our healthcare facility and local hospitals, ensuring seamless communication and coordination of patient care. You will play a vital role in facilitating the transition of patients between our facility and the hospital, while maintaining open lines of communication with medical staff, care teams, and family members.

Responsibilities:

Establish and maintain relationships with local hospitals, physicians, and other healthcare providers to ensure effective communication and coordination of patient care Verify and coordinate patient admissions, discharges, and transfers between our facility and local hospitals Facilitate the exchange of medical information, test results, and treatment plans between healthcare providers Serve as a liaison for patient families and care teams, addressing concerns and providing education on patient care and treatment plans Monitor patient flow and communicate with hospital staff to ensure timely and efficient care transitions Participate in quality improvement initiatives to optimize patient outcomes and facility operations

Requirements:

2 years of experience in a hospital or healthcare setting Excellent communication, interpersonal, and problem-solving skills Ability to work in a fast-paced environment and prioritize multiple tasks Strong understanding of healthcare operations and patient care coordination Compassionate and empathetic approach to patient care and family support
