Job Description

Inpatient Care Navigator Social Worker, Day Shift, Care Navigation Shady Grove Medical CenterIf you are a current Adventist HealthCare employee, please click this link to apply through your Workday account.

Shady Grove Medical Center seeks to hire an experienced Inpatient Care Navigator for our Inpatient Behavioral Health Care Navigation Department who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing.

As a Care Navigator you will:
    Patient Assessment:
    Conduct comprehensive assessments of patients' behavioral health needs upon admission, including mental health status, psychosocial factors, and discharge planning requirements.
      Care Coordination:
      Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and healthcare providers to ensure continuity of care throughout the inpatient stay.
        Multidisciplinary Collaboration:
        Work closely with psychiatrists, psychologists, social workers, nursing staff, and other members of the care team to facilitate effective communication and collaboration regarding patient care
          Documentation:
          Maintain accurate and timely documentation of assessments, care plans, and progress notes in compliance with regulatory standards and organizational policies.
            Discharge Planning:
            Initiate and coordinate discharge planning early in the hospitalization process, connecting patients to appropriate community resources and outpatient services post-discharge.
              Patient Advocacy:
              Serve as a patient advocate, addressing concerns and ensuring that patients and families are informed and involved in treatment decisions.
                Quality Improvement:
                Participate in quality improvement initiatives and contribute to program development aimed at enhancing the delivery of inpatient behavioral health services.
                  Crisis Intervention:
                  Respond effectively to crisis situations, providing support and stabilization for patients in distress.
                    Education and Support:
                    Provide education and resources to patients and families regarding behavioral health conditions, treatment options, and coping strategies. Qualifications include:
                    • Master's degree in social work, counseling, psychology, or a related field required.
                    • Minimum of 2 years of experience in behavioral health care management, case management, or a related field, with a strong understanding of inpatient behavioral health settings.
                    • Current licensure in a relevant field (e.g., LMSW, LPC) required
                    • Certification in care management or behavioral health (e.
                    g., CCM, PMH-C) is a plus.

                    $71,932.12 - $107,889.60 If the salary range is listed as $0 or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.
