Event Overview We are hosting a dynamic Hiring Event for Patient Continuity and Needs Advocates who are passionate about making a meaningful difference in the lives of seniors. This role offers an exciting opportunity to join a mission-driven organization committed to improving health outcomes through comprehensive case management, discharge planning, and patient advocacy. Ideal candidates will bring healthcare experience, excellent communication skills, and a genuine desire to serve diverse populations. Duties Collaborate with healthcare teams to coordinate patient care transitions Assist patients in navigating social services, community resources, and insurance options Conduct intake assessments and gather detailed patient information using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems Provide patient education on health management, medication adherence, and follow-up care by collaborating with their healthcare teams Communicate effectively with patients from diverse cultural backgrounds to ensure understanding and engagement Support in organizing utilization review and management processes to optimize patient care plans Document all interactions accurately while maintaining compliance with HIPAA regulations Skills Experience working with managed care organizations and health insurance processes Proficiency in Microsoft Office applications and EMR/EHR systems for documentation and communication Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds Experience working in Medicare a plus Customer Service Skills Empathy Self motivated and confident Ability to connect well and build trust with seniors Join us at this Hiring Event to become part of a passionate team dedicated to changing lives through exceptional healthcare advocacy!
