St. Mary's Hospital & Methodist Hospital, Rochester, MN Join a Healthcare Team Making a Difference We are seeking a Hospital Liaison to serve as the vital connection between hospital care and home healthcare services. In this role, you'll work directly with hospital case managers, physicians, nurses, social workers, and patients to ensure a smooth transition from hospital to home. What You'll Do Coordinate respiratory and durable medical equipment (DME) services for discharged patients. Collaborate with physicians, nurses, discharge planners, social workers, and case managers. Educate patients and caregivers on equipment use, maintenance, and safety. Complete insurance verification and referral documentation. Coordinate equipment delivery and setup in patient homes. Maintain accurate patient records and delivery documentation. Respond to urgent equipment needs and service requests. Build strong relationships with hospital staff and referral sources. Participate in quality improvement initiatives and compliance activities. Represent the organization at community and professional events. Required Qualifications High School Diploma or GED. Valid driver's license with a clean driving record. Ability to lift and transport equipment up to 65 pounds. Strong communication, organizational, and customer service skills. Preferred Qualifications Hospital, healthcare, or patient-facing experience. Experience with respiratory equipment or durable medical equipment (DME). Knowledge of discharge planning, case management, or patient coordination. Medical office or healthcare administration experience. Knowledge of insurance verification and medical billing. Medical terminology knowledge. Three or more years of related healthcare experience. Technical Skills Microsoft Outlook, Word, and Excel. Electronic documentation systems. Email and internet-based communication tools. Standard office technology and mobile devices. Location Requirements Must reside within 30 miles of Rochester, MN. Primary work location is on-site at St. Mary's Hospital and Methodist Hospital. Performance Bonus Opportunities Schedule:
Tuesday-Saturday, 8:30 AM-5:00 PM Experience with hospital discharge planning, patient coordination, case management, respiratory care, DME, or healthcare services is highly valued
