Deaconess Hospital | Social Workers | PRN/Casual Call Posted 7 days ago Apply Save Description The Psychiatric Services Patient Navigator assists with patients who have complex psychosocial needs and require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Participates in the development of a discharge plan of care for high-risk patient populations. Receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. All age groups are served by this position.

Minimum Qualifications:

Required:

Bachelor's Degree in Community Health, Medical Social Work, or related field Current American Heart Association (AHA) Basic Life Support (BLS) certification. Six (6) months of Community Resource experience

Preferred:

Master's of Social Work (non-licensed) One year community resource management experience

In addition to the essential functions of the job listed below, employees must maintain timely completion of all required education, trainings, and certifications as assigned and in accordance with Bozeman Health policies, department-specific requirements, applicable certification program and registry requirements, and other applicable regulatory requirements. Collaborates with the Crisis Intervention Specialist or Behavioral Health Specialist to develop and implement an individualized person-centered treatment plan. Identify needs in social determinants of health and connect the patient and family to community resources Track high frequency/at risk patients throughout the course of care and update team of potential issues Advocating for patients and their needs Helps with patient assistance programs Assists and set ups Telehealth support for patients Other related duties and responsibilities as assigned.
