GED Full Job Description Overview:
The Case Management Representative supports the implementation of care management services to select patients in the community. This position will assist patients and caregivers in navigating community resources to reduce barriers to care and improve health outcomes. As a Case Management Representative, this position will perform outreach and intake interviews to collect patient-level information. Works collaboratively with providers and clinical staff to ensure prompt coordination of care and follow up. Promotes optimal health and self-management through advocacy, empowerment, identification of and connection with resources, facilitation of efficient service delivery, and promotion of a positive patient-PCP relationship. Assists with administrative support tasks.
