Patient Relationship Liaison The Patient Relationship Liaison will facilitate care coordination from acute to ambulatory and is a valued member of the healthcare team. They collaborate with Physicians, Case Managers and other members of the healthcare team to facilitate coordination of care and follow-up appointments to the ambulatory setting. Collaborates with Case Managers for guidance with medically and/or psychosocially complex cases. Serves as a resource for care coordination. Maintains current knowledge of available community resources, post-acute care and ambulatory options.Schedule:
Monday - Friday | 8:00am - 4:30pmLocation:
Presbyterian Medical Center (Reimbursed travel to Huntersville Medical Center twice a week)What We're Looking For:
Scheduling experience within EPIC, highly preferred.Education :
High School Diploma or GED, required. 2 Year / Associate Degree, preferred.Experience :
2 years in Healthcare, preferred. Additional Skills (required): Some weekend and holiday work required.What You'll Do:
Guide patients through the transition from inpatient care to outpatient treatment by coordinating follow-up services and addressing potential gaps in care. Utilize dynamic patient census and discharge reports to prioritize outreach efforts and support effective care management planning. Collaborate with patients, providers, and care teams to arrange primary and specialty care visits, promoting adherence to post-discharge recommendations. Enhance the patient discharge experience by ensuring follow-up appointments are scheduled before discharge, improving continuity of care and patient outcomes.Benefits
